Case management

Every case, and what comes next

Assessments, settlement plans, notes and follow-ups sit on each case. Cairn shows where every client is and what to do next.

My Work: a caseworker’s follow-ups, such as reviewing a settlement plan and booking an interpreter, each with its client and due date.

A week in case management

What caseworkers do in Cairn, and what their manager sees.

  1. 1

    Assess and plan

Accept a referral and open a case. Record the eligibility assessment, then the client’s needs and goals.

  2. 2

    Keep in touch

Text appointment reminders in the client’s language. Replies are matched to the right client.

  3. 3

    Follow up and report

Set a follow-up for next week or next month. Each session feeds the right iCARE template.

Where each client is

A case stage worked out for you

Cairn reads each case’s latest assessment and shows its stage, with a one-line hint for the next step.

  • Eligible with low or high need, or not eligible
  • Re-assessment due and exit due
  • Cases with no contact in a while, flagged
A case management case open on its notes, with each contact’s date, type and note.

Your caseload

See the whole program at a glance

Managers see every case in their programs and sites, and hand cases over when someone is away.

  • My open cases, missing documents and no contact in N days
  • Change the primary worker for several cases at once
  • Restricted cases stay hidden from everyone not named
Today, with the day’s sessions, overdue follow-ups, and cases needing attention because of no recent contact or missing documents.

Ready for iCARE

Each Case Management session reports on its own IRCC template: eligibility, direct services, referrals, ongoing assessment and exit.

Cairn asks only the questions each assessment needs, and fills in answers that follow from earlier ones.

  • Eligibility results and levels of need
  • Direct services and referrals, topic by topic
  • Ongoing and exit assessment results
  • Hours, language, formats and supports for each client

Questions about case management

Can’t find what you need? Write to us and a person will answer.

[email protected]
Can a case belong to more than one caseworker?

Yes. Each case has one primary caseworker, and a program manager can assign more staff to it.

What happens when a client moves to another program?

Refer them from the case. The receiving program accepts the referral, which opens a new case there.

Can we close a case and reopen it later?

Yes. Closing takes a date and an optional reason. Reopening keeps the earlier closure in the case history.
